Excerpt from the Conejo Valley Unified School District's website announcement (last updated March 26, 2004): "Effective July 1, 1999, all students entering 7th grade will be required to present documentation of hepatitis B immunization (AB 381). Required documentation will be an immunization record showing that 3 doses of hepatitis B vaccine have been received by the student. The record must indicate the date each dose was received.
When your student has finished the series, please bring the documentation to the school office so that it can be added to his/her permanent school immunization record.
Thank you."

| Thus, parents of incoming grade 7 students will receive these documents to complete at home and return to school:

Subject: Update and Changes in Seventh Grade Hepatitis B Immunization Requirement
Pursuant to Health and Safety Code Section 120335, since July 1, 1999, seventh grade entrants have been required to have 3 doses of hepatitis B vaccine and a second dose of measlescontaining vaccine (usually given as MMR). Schools have done an excellent job in enforcing this law during its first year of implementation, which we gratefully acknowledge. We are notifying you now of some changes in the Hepatitis B vaccine, which will require some changes in enforcement of the regulations.
As of February 17, 2000, thp Food and Drug Administration (FDA) and the U.S. Public Health Service Advisory Committee on immunization Practices (ACIP) have approved an altemative new formulation and regimen of Hepatitis B immunization', which allows for a 2-dose series in some circumstances. This new regimen is licensed, however, for use oniv in 11 through 15 year-olds and currently with only one of the Hepatitis B vaccines. This option allows care providers and parents additional choice when immunizing adolescents who are preparing to meet the 7th grade entry requirements. The optioris now include the previously licensed ~dose regimen,vaccines manufactured by Merck and Co. and by SmithKline Beecham, plus the new 2-dose regimen ~th the "adult" 10 mcg/dose vaccine manufactured by Merck and Co.
Immunization providers are being given information on how to document administration of the 2dose series so schools can determine whether students entering seventh grade have been adequately immunized. The message for Fall 2000 and beyond is that schools can accent an immunization record which shows either (a) 3 doses of HeDatitis B vaccine. OR (b) 2 doses of Henatitis B vaccine and a provider's sianature notina that "Both doses were 2-dose formulation," as shown on the sample attached. This means that either the regular 3-dose scheduie or the new 2-dose schedule (if it is certified by the health care provider that the appropriate product was used for both doses) fulfills the 7th grade entry requirement for Hepabbs B.
In order for school personnel to document receipt of this 2-dose regimen on the blue Califomia School Immunization Record (CSIR), they will need to write in the two dates the vaccine was administered and then write "PROVIDER SIGNATURE" in the space where the third dose would have been documented. A sample is attached.

Immunizations
According to California State Law, to attend the seventh grade students are
to have the Hepititis B series (this is a series of three vaccinations) AND
a second M.M.R. (Measles, Mumps, Rubella) vaccinations completed. If your
student is in the sixth grade, please be sure this requirement has been met or
you are in the process of meeting this requirement. A copy of the updated
information needs to be submitted to the Health Clerk.
Most of our sixth graders are already in compliance.
